     To subscribe or to Renew and existing Subscription to the CDC Remote Access
      Service, please follow ALL steps in their respective order:

      In order to successfully complete Step 1, Adobe Acrobat Readermust be installed
      on your PC.  If you do not have Adobe Acrobat Reader installed, please click HERE
      to download the free software from Adobe's website. 

      Click the "Continue" button on the Adobe web page to start the download process.

      Step 1:  Fill out the CDC Remote Access Application Online
      Click the link below for the desired subscription in order to open the correct CDC Remote
      Access Form.  Please complete the online form and click the Submit button at the bottom.

      EXISTING SUBSCRIBERS - We ask that you please fill out the forms so we can ensure
      that we have your correct contact and location information.  We also must receive a new
      subscriber agreement as you are subscribing for a new specified amount of time.

      We must receive the executed forms in order to
      create/renew the account.

      Step 2:  Submit your payment.

      Credit Cards are the only form of payment accepted online.

      or drop off your payment to:
